Community Health Needs Assessment

In late 2021, Salinas Valley Health collaborated with five other local health organizations to establish the Monterey County Health Needs Collaborative, a partnership created to conduct a comprehensive Community Health Needs Assessment (CHNA) for Monterey County.

Now in 2025, we are excited to be a part of this project once again. Similar to the previous assessment, this evaluation will serve as a means for our organizations to unite in identifying and improving the health needs of our communities.

2022 Monterey County Health Needs Collaborative

The 2022 assessment includes months of primary and secondary data collection, incorporating extensive input from the community and experts in public health, clinical care and others. This CHNA process and report comply with current federal requirements and have served as a basis for implementing strategies by the Healthcare System and Collaborative partners to address community needs.

Individual ratings for each criteria were averaged for each tested health issue, and these composite scores were averaged to generate an overall score. This process yielded the following prioritized list of community health needs:

  • Mental Health
  • Access to Health Care Services
  • Nutrition, Physical Activity and Weight
  • Heart Disease and Stroke
  • Substance Use
  • Housing
  • Infant Health and Family Planning
  • Injury & Violence
  • Cancer
  • Oral Health
  • Potentially Disabling Conditions
